Dear Ministry of Social Affairs Team,
I am Dr. Gafur Jafarli, a physician with 30 years of clinical experience and leadership as a CEO and owner of private hospitals. I hold professional certificates in cardiology, internal medicine, neurology, endocrinology and phlebology obtained in Azerbaijan
and Russia, including Baku and Moscow. I am also a full member of the European Medical Association in Belgium and hold its certificates.
I would welcome cooperation with Estonian clinical and digital health partners on the SNB Global Health Passport.
We propose a 12-month pilot involving 1,000 patients with hypertension and a separate cohort of 1,000 patients with type 2 diabetes. Each participant would be counted once.
The pilot would assess integration with existing records and the value of connecting personal risk assessment, physician-led care plans and systematic follow-up of abnormal findings. AI would assist clinical teams; locally licensed physicians would lead care.
Independent evaluation would measure clinical control, follow-up completion, clinician workload and total costs. Our ambition is to test a 20% reduction in avoidable chronic-care costs, including programme costs, compared with usual care.
My purpose is to contribute clinical experience to a scalable public-health service. I seek no personal award or honorarium and propose that an agreed share of verified net savings support sustainable delivery and wider access.
I would welcome an introductory meeting to identify a local partner and develop a jointly costed pilot consistent with GDPR and Estonian clinical and regulatory requirements.
Kind regards,
Dr. Gafur Jafarli
